5 Tell-Tale Signs It’s Finally Time for a Knee or Hip Replacement

The 2023 American College of Rheumatology and American Association of Hip and Knee Surgeons guideline conditionally recommends proceeding with joint replacement without delaying for additional nonoperative treatments when people with moderate-to-severe osteoarthritis or osteonecrosis have already been indicated for surgery and have not improved with nonoperative therapy.

However, modern medical guidelines from the American College of Rheumatology (ACR, 2023) show that delaying necessary surgery to pursue endless conservative treatments does not improve long-term outcomes and only prolongs your suffering. What are the signs you need a knee or hip replacement?

The clearest signs that you need a joint replacement include persistent pain that keeps you awake at night, severe limitations in your daily mobility, and joint damage that no longer responds to conservative treatments. When these symptoms interfere with your quality of life despite treatment, it may be time to discuss whether joint replacement is appropriate. 

Here are the five key signs to watch for:

1. Conservative treatments no longer provide relief

If you have tried arthritis treatment, joint injections, and targeted physical therapy and still have severe pain that limits your daily life, it may be time to discuss joint replacement. When the protective cartilage wears away gone, nonsurgical options can no longer restore smooth movement.

2. Joint pain is changing the way you move

When you limp to protect a painful knee or hip, you alter your natural walking pattern. This compensation forces your lower back and opposite joints to absorb abnormal loads. 

In a 2026 Cleveland Clinic study of 4,412 Medicare patients undergoing total hip replacement, those who also had back and opposite-hip pain before surgery were more than three times as likely to have less meaningful functional improvement at one year. That’s why it’s important to look at the whole picture — not just the painful joint — when deciding on the right treatment plan.
