At Integrity Orthopedics Physical Therapy, we use an integrated Team Practice approach working with your Surgeon to safely and effectively improve orthopedic conditions for the whole body. Everything from Neck and Back Pain, Shoulder Pain, Total Hip Replacements, General Hip Pain, Knee Pain, Total Knee Replacements will improve with our approach. Our clinic provides a hands-on approach to ensuring to achieve full muscle length after injury or surgery and increase strength gains to your level of active lifestyle. The team of therapists is guided by over 30+ years of experience with the Classens in the rehabilitation industry and include multiple Doctors in Physical Therapy, Physical Therapy Assistants, Exercise Trainers and an amazing Front Desk Team. We are here to help you improve your function. 

Physical Therapy Expectations

Our phone line goes directly to the Physical Therapy Department and will not connect to the Medical Office of Dr. Muradov. If you have any needs for physical therapy please be sure to call us.

To make an appointment with out team we will need some information from you. We will send electronic forms to be filled out before your evaluation can be completed. If you are unable to do them electronically you are welcome to come into the office to complete them before your appointment day. This will allows us to verify insurance benefits, collect doctor's information (if not seeing Dr. Muradov), update file information and make sure your visit is handled smoothly. We do ask all evaluations to have a card on file as part of our company policy. Once all forms and information is collected you can arrive to your visit ready to be seen.  If you are an existing patient we may ask you to sign forms if we have not seen you in 6 months. We ask you to arrive 5-10 minutes early for your appointment. Please call or email us with any patient concerns.
